Informed Consent Form

I, [Client's Name], hereby consent to participate in Energy Healing sessions with Dochka Hristova. I acknowledge that it is my responsibility to thoroughly review the following information:​

 

Nature of Energy Healing:

 

Energy Healing is a gentle and peaceful practice that works with the body’s energy system to support healing and help to restore wholeness and health. Energy Healing is not a substitute for medical diagnosis, treatment, care, or counselling.

 

Explanation of Process:

 

The sessions will be conducted online via a secure video conferencing platform. I will be fully clothed and participate from a comfortable and private location of my choice. Dochka will guide me through the healing experience. The process has been explained to me, and I have been encouraged to ask any questions.

 

​Confidentiality and Recording:

All sessions and personal information shared during Energy Healing sessions are strictly confidential. For confidentiality and safety reasons, sessions are not recorded. I agree to be fully present and not to record sessions in any way, although I may take written notes for personal reference if I wish. Dochka will only disclose my personal information with my consent or as required by law.

 

Limitations and Risks:

 

Energy Healing is generally considered safe; however, it may bring about emotional releases or temporary discomfort as unresolved issues surface. I may experience sensations such as warmth, tingling, or fatigue during or after the sessions.​

Payment and Session Duration:

Payment of the full fee is required at the time of booking via a bank transfer to confirm my appointment. Sessions may vary in length, typically lasting between 30 and 90 minutes.

 

Cancellation and Rescheduling Notice:

 

48 hours' notice is required for appointment cancellations or rescheduling. Failure to provide this notice will result in the forfeiture of the paid fee.​

 

No Guarantees:

 

I acknowledge that healing outcomes can vary, and there are no guarantees made by Dochka regarding the specific results of Energy Healing.​

 

Complaints Procedure:

 

I note that all concerns or complaints must be emailed to Dochka within 21 days of any issue occurring. Dochka is dedicated to resolving any issues promptly and effectively. As Dochka is based in the UK, all matters will be governed by UK law.
